Safety rules

Five rules, evaluated on every MedicationRequest write and on hourly scheduled scans.

MAJOR blocks
Drug-Drug Interaction
Checks all active MedicationRequests for the golden patient across ALL branches against NLM RxNorm API before every prescription write. MAJOR = write blocked until prescriber overrides.
Blocks on match
Drug-Allergy
Checks new MedicationRequest against all AllergyIntolerance resources for the golden patient across all branches. Cross-branch allergies are visible because they share the same golden ID.
Alert only
Missed Follow-Up
Hourly scan for Observations with OBX-8 = H or L older than 72 hours with no follow-up Encounter. Alerts the ordering clinician and branch admin. Window configurable.

Polypharmacy
Flags patients with >10 active MedicationRequests across all branches and age >65. Alert goes to primary clinician for medication review. Thresholds configurable.

Duplicate Prescription
Detects same ATC drug class prescribed from two different branches within 30 days. Prevents independent double-prescribing across a hospital chain.
